Identification

To readily identify the type of Consent Form, place one of these labels at the top of the first page: “Adult Informed Consent Form,” “Parental Informed Consent Form,” “Legally authorized Representative Informed Consent Form.”  Additionally, the IRB protocol number should be placed immediately above the title of the study as soon as a number is assigned
