Financial Obligations
The Financial Obligations section of the consent form is required when the research involves a procedure for which there is a fee.

Financial Obligations

The consent form must state clearly whether or not the subject will incur any financial obligations as a result of participating in the study and the extent of the obligation (for example, a fee for a cholesterol test).
