If you are recruiting or accessing subjects through other institutions or organizations (for example, schools, day care and nursing home facilities) not under the control of the university, your IRB application should clearly identify these institutions. Your IRB application also should describe the method for ensuring that the authorized official of the institution is informed of the study. Your application must include a letter from the authorized official on the letterhead of the cooperating institution. The letter granting you permission to conduct the study should include the authorized official’s title, contact information, and his or her official signature. If research participants will be placed at more than minimal risk, documentation is required that the authorized official understands the risk and approves the study.
