As of January 15th, employees who elected to receive their W-2 via Firefly were able to access their 2020 W-2 document by logging into Firefly>Employee Self Service and clicking on the W-2 form tile.

Paper W-2 forms have been sent directly to employee’s permanent addresses listed in SAP/Firefly for distribution to those employees who either elected to receive a paper W2 or did not make an election for their W-2 method in Firefly.  Please contact Payroll at 865-8184 if you have questions regarding W-2 documents.

Please consider donating to the crisis leave program.  This program allows any employee who earns vacation leave to donate up to five accumulated vacation days per calendar year.  The donated leave goes to a pool of crisis leave hours that are used to help an employee who is experiencing a crisis and does not have sufficient leave to cover their absence period.  Donation forms can be accessed through SAPPHIRE or the following link:

Please submit completed forms to the Human Resources Office.

A list of campus building supervisors and building abbreviations/addresses can be found at: Facilities>Building Information>and select either Supervisor List or Building Abbreviations and Addresses.

Since FedEx and UPS deliver directly to your office, please make sure to change the default address of 905 West 25^th in eSHOP to your department address.

If your department has moved to a new location on campus or your address is not one of the selections available, please fill out a Delivery Address Request in eSHOP.  When Kim Christensen has received the form, she will make the changes in SAP, which will be available the day day in eSHOP.

Adding A Ship-To Address To Your Profile

You may add any of the Ship-To Addresses in the system to your profile, and make one of them your default: 

1. From your profile, under Default User Settings, click on Default Addresses.
2. Click on the Ship-To Tab.
3. Click the Select Addresses for Profile button.
4. Search for the appropriate address and then Click in the Radial Button next to the Address you wish to add to your profile.  Preset Addresses for UNK start with UNK (space)-(space) Department Name (Example:  UNK-Procurement

Adding An Unloading Point To Your Profile 

The Unloading Point is a required field that tells the supplier exactly where the item should be sent:

1. From your profile, under Default User Settings, click on Custom Field and Accounting Code Defaults.
2. Click on the Header (ext.) Tab.
3. Click the Edit button and Create New Value.
4. Enter the Unloading Point in the Value Field. This should be the Building Abbreviation and Room Number.
5. Check the Default checkbox to make this your default.
6. If necessary, repeat the process to add additional Unloading Points.
7. Click the Save button when you are finished.

More than 400 Americans die from unintentional carbon monoxide poisoning every year, according to the Centers for Disease Control and Prevention. More than 20,000 visit the emergency room, and more than 4,000 others are hospitalized.

Carbon monoxide is an odorless, colorless gas that often goes undetected, striking victims caught off guard or in their sleep.

This "silent killer" is produced by burning fuel in cars or trucks, small engines, stoves, lanterns, grills, fireplaces, gas ranges, portable generators or furnaces. When the gas builds up in enclosed spaces, people or animals who breathe it can be poisoned. Ventilation does not guarantee safety.

Carbon monoxide detectors save lives, but less than one-third of American homes have one installed. With December and January at the peak of CO poisonings, be sure to follow tips like these:

• Replace the battery for your home’s CO detector each spring and fall
• Do not heat your home with a gas range or oven
• Never run a car or truck inside an attached garage

Never ignore a carbon monoxide alarm, and do not try to find the source of the gas. Instead, follow these steps:

• Immediately move outside to fresh air
• Call emergency services, fire department or 911
• Do a head count to check that all persons are accounted for
• Do not reenter the premises until emergency responders have given you permission to do so
