Students must notify the Office of Student Records and Registration of changes in their Social Security Number or Student Identification Number.
The changes can be made by
1) Completing the Change of Social Security form below and attach a copy of the Social Security card mail it to:

2) Completing the Change of Social Security form in the Office of Student Records and Registration located in Founders Hall.
Students changing their social security number must present their social security card so a copy of the card can be made.
