As a patient at UNK Health Care you have the right to:
- access care and treatment
- consideration, respect and dignity
- confidential handling of records and disclosures
- an explanation of your health problem and treatment
- refuse any tests or treatment to the extent permitted by law and be informed of the medical consequences of your decision
- suggest changes in policy or service delivery by submitting your ideas to UNK Health Care
- express dissatisfaction with services received by contact with the Health Care Director or using any other resources available through UNK administration
As a patient at UNK Health Care you have the responsibility to:
- submit a completed health record form to UNK Health Care
- provide, to the best of your knowledge, accurate and sufficient information about your medical history
- follow your diagnostic and treatment recommendations
- accept personal responsibility if you refuse care
- abide by Health Care regulations and policies
- assume financial obligation for services you receive from UNK Health Care, or a referral clinic or hospital
- respect the rights of other patients and UNK Health Care staff
