The following forms should be completed prior to participating in a UNK Faculty-led program abroad.
This on-line form should be completed by each UNK student participating in a Faculty-led Short-term International Program.
Short-term Abroad Disclaimer using Agent (downloadable)
This form should be signed by each UNK student participant in a faculty-led program abroad when using a tertiary agent (i.e., EF Educational Tours, Passports Educational Group Travel, etc.). All forms should be collected by the Program Director and returned to Ann Marie Park (Welch Hall #104) two weeks prior to departure from the U.S.
Short-term Abroad Waiver, Release, and Indemnification Agreement (downloadable)
This form should be signed by each UNK student partipant in a faculty-led program abroad when not assisted by an agent. All forms should be collected by the Program Director and returned to Ann Marie Park (Welch Hall #104) two weeks prior to departure from the U.S.
