The Consent Form should be long enough to explain the consent factors adequately, but not so lengthy or detailed as to lose the attention of the subject or cause confusion.
If the Consent Form in longer than one page, provide signature or initial blanks on each page to ensure that informed consent is begin given for all of the elements of informed consent contained on that page.
