The agency supervisor is to complete this form to evaluate the performance of the student at the completion of the PE 895 Internship. This report should be reviewed with the student and sent to the college supervisor within five (5) calendar days after completion.

Initiative: The extent to which the student displays the physical and mental energy, imagination, resourcefulness, spirit, and drive required to get results without the need of urging on the part of his supervisor. 5 3   2 1 N/O
Loyalty: The degree of support the student gives to his/her fellow workers, superiors and the program of the agency. 5 3   2 1 N/O
Attendance and Promptness: This refers to the use of sick leave, unauthorized absences, and promptness in reporting for work. 5 3   2 1 N/O
